From St. Paul take 35E N out of the city. 35E N will merge and become I-35 N. Take the MN 97 (CR-23) exit (EXIT 129). Go Right onto MN 97 approximately 1.9 miles. MN 97 becomes Forest Blvd or Highway 61. Turn right onto Scandia Trail (MN 97) for .2 miles. Clinic is located across the street from the Forest Lake High School on the left.

From Minneapolis take 35W N merging onto 35E N. Take Take the MN 97 (CR-23) exit (EXIT 129). Go Right onto MN 97 approximately 1.9 miles. MN 97 becomes Forest Blvd or Highway 61.
